Our Support Coordination helps people with disability and their family/support network to navigate the different stages of their National Disability Insurance Scheme (NDIS) journey and achieve their goals.


Our qualified and experienced Support Coordination team can support you to understand and implement your NDIS plan. We aim to build your capacity for independence and the management of your own supports.

Our Support Coordination include:

Arranging healthcare appointments such as Occupational Therapy, Physiotherapy and Psychology

Understanding participant needs

Managing relationships between supports and participants and their families/carers

Non-judgemental service provision

Tailor supports specific to participant

How we can help you

We will work closely with you and your support network to support you to achieve your NDIS goals.


We will:

Identify the services that best suit your individual needs

Obtain quotes for services and set up Service Agreements with providers

Monitor  your progress towards your NDIS goals and the performance of the support services you use

Request a plan review if your plan does not meet your needs, and

Review your goals prior to your NDIS plan review
